As the band website is currently under maintenance, I have uploaded the official leave form for download. Click here to download the leave form.
If you are going overseas this holiday, and you cannot find the hard-copy version of the leave form in the band room, you can now download the soft-copy version and fill in the leave form before you go for your holidays.
